How to avail Vitality Fitness Assessment (VFA)?

1. Download the VFA form

   Accomplish the VFA form, downloadable from the app.

2. Book an appointment

   Advance booking is required. Set an appointment with UFC Gym or fill out the online booking form in the AIA Vitality app.

3. Present your e-Card

   Show your e-Card along with one (1) valid ID on assessment day.

4. Automatically earn points

   Points will automatically be reflected 3 to 5 days after assessment date.
